Travel Agency "Municipal-Odessa" Ltd. has been appointed as Co-organizer and Official Travel Partner for ISMH 11 and will handle all registration and accommodation arrangements.

The registration form should be completed for all delegates, including speakers. Registration will only be valid after payment has been received

The deadline for early registration is 30 April 2011.

Registration fees:

*Students must submit the official document verifying their status with the registration form and full payment; registration will not be processed until the official notification is received.

The fee for participants and students includes:

Participation in the symposium scientific sessions, poster display, symposium kit, participation in the opening ceremony and get-together party, Gala-dinner, coffee-breaks, a certificate of participation, transfer.

The fee for accompanying persons includes:

Participation in the opening ceremony and get-together party, Gala-dinner, social events, transfer.

Cancellations and refunds.

Cancellations and reimbursement if the cancellation is made:

a)                       before 31 March 2011 – total refund minus bank charges

b)                      before 30 June 2011 -  50% of the registration fee minus bank charges may be paid back.

After 30 June 2011 – no refund will be possible.

Only written cancellations which reach the travel agency before the above dates can be acknowledged as the valid cancellation.
